Kunsmiller Middle School What Services are offered by the
School, sports, camp, and employment physical exams Immunizations Routine lab tests Prescriptions and medications Care for acute injury and illness Care for common concerns such as acne, menstrual and weight problems Pregnancy testing Testing and treatment for sexually transmitted diseases Birth control information Follow-up as requested by primary care provider Coordination of medical and psychiatric conditions Individual, group and family mental health counseling Drug and alcohol education and counseling Parent and student health education Vision and hearing screening What Services aren't provided?
Hospitalization X-rays Non-routine lab tests Dental and vision care Prescribing or dispensing of contraceptive devices Abortion counseling Insurance Enrollment Families are strongly encouraged to provide health insurance information, and to apply for low-cost child health insurance options such as Medicaid and Child Health Plan Plus when their children are uninsured. Staff is available through the SBHC to assist with the application process. Parent Involvement? A parent or guardian must sign a consent form before a student may use any health center service. Once this is done, the student may use the health center at any time. Supporting family communication is a principal goal of the health center. Staff encourages students to discuss their health care with their parents. However, parents are not routinely notified when a student uses the health center, except by student request or when the staff becomes aware of serious health concerns.
Confidential... As in the case of all health visits, student visits to the health center are confidential. Information is not shared with anyone else without the student's and/or the parents' permission. The only exceptions are life-threatening situations.
What Does It Cost? There is no charge for medical services to the students or family. Students' will not have to pay co-pays, deductibles or fees to be seen. There may be a charge associated with mental health and substance abuse services when students health insurance does not pay for the service.
